For IEHP, Please call the number that is on your IEHP insurance card and request an *authorization number for the office of "Norina Murphy & Associates". Once you receive the authorization number that starts with an "H" for our office, you may fill our form out on the "Contact" Tab or call/text us with the authorization number.

* An authorization does not guarantee an appointment, screening will still need to be completed.
